Atletico was hopeful the 47-million-euro ($67 million) signing would recover from a leg muscle tear to play at the Santiago Bernabeu stadium, but Manzano ruled out the Colombian international on Friday.
Manzano says “he is not 100 per cent and it's not worth the unnecessary risk when there is so much season ahead of us.”
Falcao has scored eight goals in all competitions.
Defenders Antonio Lopez and Silvio and midfielder Tiago are also out because of injury.
Ninth-place Atletico trails Spanish leader Madrid by 15 points and hasn't beaten its crosstown rival in 12 years, a run of 21 games.
