The Spanish club said on Monday that the 31-year-old midfielder will have to undergo surgery but did not say how long he is expected to be sidelined.
Spain striker Fernando Llorente is also doubtful for Bilbao's home game against Atletico Madrid on Thursday due to tendinitis in his left knee.
This is Gurpegui's fifth season with Bilbao since moving from his boyhood club Osasuna.
Bilbao is currently 12th in the Spanish league.
